148. évfolyam (2011) Supplementum I. 25 A scleramegtámasztás (sustentaculum sclerae) elmélete és gyakorlata Berta András Debreceni Egyetem О ЕС, Debrecen, Szemklinika fisz ESCRS és az EURORETINA 2010 szeptemberében együtt tartotta kongresszusát Párizsban. A két Társaság egy közös szimpóziumot rendezett „Myopia Lens and Retina” címmel, melyben refraktív sebészek és retinaspecialisták tematikus előadásokban áttekintették a myopia epidemiológiáját, etiológiáját, genetikai hátterét, a myopia korrekciójára alkalmazható műtéteteket, a myopiás maculaelváltozások kialakulását - látásrontó következményeit és azok kezelésének lehetőségeit, valamint a myopiásokon végzett lencse- és refraktív sebészeti műtéteknek az retinaleválás kockázatát fokozó hatását és a megelőzés lehetőségeit. Mivel az előadásokban, myopiaellenes műtétként, kizárólag lencsén és corneán elvégzett beavatkozásokról beszéltek, a szekció végén megtartott vitában három kérdést tettem fel az előadóknak és a közönségnek: 1. Milyen műtétet vagy műtéteket lehet végezni progrediáló nagyfokú myopiában? 2. Van-e olyan műtéti eljárás, amivel a progrediáló nagyfokú myopia progressziója megállítható? 3. Mit tegyünk a progrediáló nagyfokú myopiában szenvedő betegeinkkel? Miután erre a három kérdésre nem kaptam megnyugtató válaszokat, elhatároztam, hogy áttekintem a Magyarországon a scleramegtámasztással kapcsolatban megjelent közleményeket, disszertációkat és az általunk és mások által végzett ilyen műtétek tapasztalatait. Végül felvetem a kérdést, hogy napjainkban, a korszerű refraktív sebészeti műtétek birtokában, van-e létjogosultsága a scleramegtámasztás vagy más myopia progressziójának megállítását célzó műtétek végzésének. The theory and the practice of scleral reinforcement (sustentaculum sclerae) András Berta University of Debrecen, Debrecen, Department of Opthalmology In September 2010 the ESCRS and the EURORETINA held their congresses together in Paris. The two Societies organized a joint symposium with the title ’’Myopia Lens and Retina”, in which refractive surgeons and retina specialists, in thematic lectures, reviewed the epidemiology, ethiology, genetic backgorund, surgical methods for the correction of myopia, the development vision threatening consecvences and treatment of myopic macular pathologies, as well as the increased risk for retinal detachment, and possibilities of prevention before or after lens or corneal surgeries performed on myopic patients. As only operations performed on the lens or on the cornea were dealt with in the lectures, during the discussion at the end of the session, I asked three questions from the lecturers and the audience: 1. Is there an operation (are there operations) that can be performed in progressive high myopia? 2. Is there a surgical procedure with which the progression of progressive high myopia can be stopped? 3. What to do with patients suffering from progressive high myopia? As I did not get sufficient answers for these three questions, I decided to review the publications and PhD theses written in Hungary, as well as to evalute the experiences gained with scleral reinformecents performed by us and by others. At the end of my lecture I raize the question: is there a need and justification to perform scleral reinforcement or other operations, aiming to stop the progression of myopia, nowadays in the era of modern refractive surgical methods?
