Z a j а е ъ М.: Edema of the transplant caused by a simple vitreous hernia

The anterior membrane of the vitreous may remain intact in a part of cases after keratoplasty performed on aphakic eyes. But the condition generally regarded favourable may be the source of delayed complications. Vitreous body with an intact anterior membrane and with an unchanged volume may be herniated into the anterior chamber causing the opacity of a corneal transplant. Out of 21 cases, where the membrane remained intact, in three the corneal transplant became opaque as a result of a delayed corneovitreal contact. Spontaneous considerable herniation evolved in two cases in a period of 3—4 months, and in a case in a year after keratoplasty. To avoid the complication it is advisable to decrease the volume of the vitreous body in all transplantation eases. Another answer to the question is an extracapsular extraction where the posterior capsule of the lense may hinder the herniation of the vitreous body.

M. Z a j á c z: Über das durch einfache Qlaskörperhernie verursachte Transplantatödem

In einem Teil der an aphakischen Augen durchgeführten Keratoplastiken bleibt der M. hyaloidea intakt. Dieser im allgemeinen als vorteilhaft betrachtete Zustand, kann aber später die Quelle von Spätkomplikationen sein. Der Glaskörper mit intaktem Grenzblatt und nicht verringerter Masse kann jederzeit in die Kammer eingeklemmt (Kammerherniation) werden, wodurch das Ergrauen des Transplantats verursacht wird. In 3 der 21 Fällen, in denen der M. hyaloidea intakt blieb, entwickelte sich wegen des sich spät ausgebildeten korneovitreaíen Kontakts ein Transplantatödem. In 2 Fällen entstand die spontane, hochgradige Herniation zwsichen dem 3—4. Monat und in I Fall 1 Jahr nach der Keratoplastik. Im Interesse der Vorbeugung dieser Spätkomplikation empfiehlt es sich die Masse des Glaskörpers in allen Fällen zu verringern. Als andere Lösung kommt die extrakapsuläre Extraktion in Frage, wo durch die zurückgebliebene hintere Kapsel die Vorwölbung des Glaskörpers verhindert wird.
