Who Will Pay You If You're Sick Or Hurt? DISABILITY INCOME PLAN Non-Cancellable and Guaranteed Renewable To Age 65 With Guaranteed Premiums Individual Coverage This plan pays an income to the Insured while disability as the result of accident or sickness prevents him from performing the duties of his regular occupation. Benefit durations, elimination periods and optional extra benefits are available according to occupational classification. The minimum monthly income is $100. Larger amounts are available in increments of $50 monthly up to a maximum determined by the average monthly income. Each $100 of monthly income benefit provides the following: Total Disability — $100 per month while the Insured is prevented by sickness or accident from performing the duties of his regular occupation. Partial Disability (if elected) — $50 per month while the Insured is prevented by accident or sickness from performing one or more important duties of his regular occupation following a period of total disability for which a total disability income benefit is paid. Limited Travel Accident Indemnity— $100 per month in addition to the total disability income benefit and $50 in addition to the partial disability income benefit when disability results from an accident while riding as a fare paying passenger in a plane or public conveyance provided by a common carrier, but not to exceed three months for both total and partial disability. Principal Sum (if elected) — $1,000 for each $100 of total disability monthly income for loss as the result of accident of life, both hands, both feet, one hand and one foot, or the sight of both eyes; $500 for the accidental loss of either hand or either foot; $333 for the accidental loss of the sight of one eye; or $250 for the accidental loss of the entire thumb and index finger of either hand. Only one indemnity, the largest, will be paid for one accident. Hospital Indemnity (if elected) — $100 per month, in addition to the total and limited travel accident benefits, when the Insured is hospitalized, beginning with the first day and continuing while confined not to exceed 120 days. Waiver of Premium—Premiums will be waived during any period of disability which exists for at least 90 days, after which any premiums paid during the 90-day qualification period will be refunded. Benefits may be renewed after age 65 if the Insured continues in regular full time employment pays the required premium at age 65 and the premium is accepted by the Association. The benefit period after age 65 shall be for one year regardless of the benefit duration in force prior to age 65 and the elimination period may not be changed. Exclusion: This contract does not cover pregnancy, war or act of war, I am pleased to present to our membership the five leading producers of this Agency. The Insured may return the contract to the Association within ten days after delivery and receive a refund of the premium paid. This plan is not available to housewives or self-employed females. Single females employed on a regular basis away from home will be considered at the rates for males for the next higher occupational classification. For example, a female rated AAA by occupation would require a male A A rate. A female who qualifies for benefits at the time of issue but subsequently changes occupation to one which at the time of claim does not require gainful employment on a full-time basis away from her place of residence, the monthly indemnity for total and partial disability benefits shall be reduced to 25% of the original amounts. Features: No occupational prorating on males. House confinement not required. Incontestable after two years. Ninety-day grace period. I have the assurance of Agency Manager Leo Garshell, and his men that they shall beat the above mentioned record in 1969, and I believe they shall do it. We shall introduce our men from time to time so that the membership shall become acquainted with the men in the Field.
