FRATERNITY B Y=L AW S (The Fourth Continuation) Should the member cease to have an adult life insurance certificate in force, his membership in the Sick Benefit Department is automatically cancelled, except a) if the insured pays 35 cents monthly membership dues; or b) if he holds a sick benefit certificate issued after 1960 in which the age of term­ination exceeds age 65. Such member may continue his membership in the Sick Benefit Department even though his life insurance certificate has become paid-up or matured. 112. Application for a Sick Benefit Certificate shall be made upon the form of application blank prescribed by the Federation. The term of mem­bership shall date from the day when the Secre­tary shall record the name of the accepted appli­cant in the official register of the Federation, together with the receipt of at least one month’s dues. 113. Women members holding certificates is­sued before 1960 shall be entitled to receive sick benefit of five dollars ($5.00) per week only. The increased Sick Benefit Plans “A” and “B”, introduced in 1961, are issued only to men (males) ages 16-55. 114. The monthly dues for sick benefit insur­— 53 — ance are due and payable on the first day of each month, and if not paid within the grace period, the members shall be suspended from all benefits. A suspended member may be reinstated within three months upon the payment of all monthly dues. If the suspended member is not reinstated within three months, his name shall be stricken fiom the membership roll. 115. No sick benefit shall be paid to a member: a) unless regularly attended by a physi­cian: M. D.; b) who refuses to follow the advice and prescription of his physician; c) who within six months after his ad­mission to membership suffers from cancer, nervous or mental disease, alcoholism or any other illness the origin of which can be traced to the above-mentioned period; d) on a certificate issued by a chiroprac­tor, osteopath, neuropath, dentist or midwife. The Federation reserves the right to reject any medical certificate and order a re-examination by another physician (M. D.) at the expense of the Federation.
