FRATERNITY 69. For the admittance of a regular member, the recommendation of a member or an officer of the branch or any person officially authorized to solicit membership is necessary, together with a physician’s certificate where required, proving the insurability of the applicant to the satisfaction of the Federation. The physician’s certificate shall be filled out by the appointed physician at the expense of the Federation. No women in pregnancy shall be accepted into membership until three months have elapsed for such confinement. 70. The properly signed and completed ap­plication shall be sent to the Secretary of the Federation. The Medical Director shall pass upon such application. Applicants approved by the Presi­dent and Secretary shall be received into member­ship in accordance with existing by-laws and such membership shall be effective from the date his name is enrolled on the register of the Federa­tion provided he has already paid at least one month’s dues either to the Home Office or to the respective branch representative. 71. The member may pay his dues in advance. In such cases he shall receive the discount indi­cated on the face of his certificate. On older cer­tificates where the discount is not indicated the member shall receive: one percent for quarterly, two percent for semi-annual and three percent for annual advance payments. — 37 — 72. The monthly dues are due and payable on the first day of each month and must be paid on or before the 30th day following said date. The member who fails to pay his dues by the end of the 30-day period shall be suspended from membership rights by the office of the Controller on the 31st day after due date. He shall retain his right only to the withdrawal equity fixed in his certificate. The limit of a member’s suspen­sion shall be a maximum of three months. If during this time he pays all his arrearages, he shall be reinstated to membership. The suspended member shall inform the branch which form of withdrawal equity he desires. The branch shall report the member’s desire to the Home Office. Should the member make no choice, he auto­matically shall be placed on extended insurance. 73. Members stricken from the rolls or placed on extended insurance may be reinstated to mem­bership within five years from the date of such action if written application is made therefor, accompanied by a physician’s examination report certifying insurability, obtained at their own ex­pense. All arrearages of assessments and interest thereon at four percent shall be paid by the member. 74. A member who desires to change his mem­bership from one branch to another shall make such request either to his own branch or to the Home Office. Upon his request a transfer form — 38 —
