Kongresszusi összefoglalók Introduction: Meeting a case of optic neuritis, the most often working diagnosis is multiple sclerosis. Retrobulbar neuritis cannot be only a sign of a primary central nervous system disease, but can be a part of a systemic autoimmune disease, causing secondary central nervous system manifestation. We present the differential diagnostic opportunities, diagnostic procedures and the therapeutic diversity through our cases. Patients and methods: We present here a cohort of cases appearing in our uveitis ambulance, representing the differential diagnostic variability of the background. One case of Devic syndrome, a case of Devic spectrum disease (relapsing idiopathic optic neuritis), a case of sarcoidosis, two cases of idiopathic small vessel arteritis, two cases of morbus Bechterew, and a case of chronic lymphoid leukaemia. In five cases of them the previous diagnosis was sclerosis multiplex. Conclusion: In the diagnosis of the background of retrobulbar neuritis orbital and cranial MRI alone is insufficient, MRI of the whole myelon needed as well, and serological or liquor diagnostics. In the case of incomplete diagnostic criteria means Devic-spectrum disease. The absence of demyelinated lesions does not exclude early multiple sclerosis. Showing lesions of the white matter doesn't mean sure multiple sclerosis; the condition needs furthesr diagnostic evaluation toward excluding cerebral manifestation of systemic autoimmune diseases. Treatment and prognosis depends on proper diagnosis.
