Deep anterior lamellar keratoplasty - method and results

Purpose: to review the introductory period of deep anterior lamellar keratoplasty (DALK) at our department. Method: retrospective summing up of the experiences and prospective follow-up of the patients.

Result: DALK following Anwar’s technique has been performed since December 2006 at our department. No special technical equipping is needed. If the separation of the Descemet’s membrane is achieved, no more technical difficulty is expected. If it is not achieved, transition to different methods, including penetrating keratoplasty is possible without difficulty. Optical results with DALK are comparable to those with penetrating keratoplasty, safety and endothelial cell count results after DALK surpass those with penetrating technique.

Conclusion: even in the introductory period, DALK following Anwar’s technique does not run a raised risk, but offers advantages.

Célkitűzés: Az endothelialis lamelláris keratoplastica osztályunkon való bevezetésének áttekintése.

Módszer A tapasztalatok retrospektív összegzése, a betegek prospektiv követése.

Eredmények: 2006. decembere óta alkalmazzuk osztályunkon az endothelialis lamelláris keratoplastica Descemethántással és mikrokeratom alkalmazásával járó módszerét. Műtéttechnikai nehézséggel egy eset kivételével nem szembesültünk. A transzplantátum egy eset kivételével azonnal, egy esetben ismételt levegőbefúvást követően a helyén maradt. A szaruhártya az esetek felében tisztult fel. Egy szemen perforáló technikával ismételt keratoplasticát végeztünk, sikeresen.

Következtetés: Az endothelialis lamelláris keratoplastica Descemet-hántással és mikrokeratom alkalmazásával járó módszerének alkalmazása még a bevezető periódusban sem jelent a betegre nézve fokozott kockázatot. Esetleges eredménytelensége esetén megismételhető vagy más módszer választható.

Descemet stripping automated endothelial keratoplasty - method and results

Purpose: to review the introductory period of Descemet stripping automated endothelial keratoplasty (DSAEK) at our department.

Method: retrospective summing up of the experiences and prospective follow-up of the patients.

Result: DSAEK has been performed since December 2006 at our department. No technical difficulty was encountered except in one case. The graft remained attached in all but one case, where a repeated air injection solved the problem. The cornea cleared in half of the cases. One eye underwent repeated keratoplasty, with penetrating technique

Conclusion: even in the introductory period, DSAEK does not run a raised risk. In case of graft failure a repeated keratoplasty with the same or perforating technique is possible.
