Methods: Using a special software program, the drusen-covered area was determined from digital fundus photographs of 218 eyes, in two different-sized areas centred on the fovea (with diameters of 1000 pm and 6000 pm). The results were tested for correlation with the ETDRS visual acuity and with the foveal sensitivity. Results: Visual acuity and foveal sensitivity correlated with the drusen-covered area of the 6000 pm diameter area (R=-0.17, p=0.02; R=-0.20, p=0.006, respectively), but not with that of the 1000 pm area (p>0.05). Conclusions: Our findings strongly suggest that visual impairment in early AMD depends on ophthalmoscopically undetectable dysfunction of the RPE/photoreceptor complex, rather than on the presence of drusen in the fovea. This arises not only because of the ethical implications, but also because of the proliferation of court cases. Some of these issues are: • How detailed should the counselling be, and which potential complications should be discussed? • How much time should be spent on counselling? • Should a witness be present? • Who has the hnal decision regarding treatment details? • Should the physician agree to all decisions made by the patient? All these issues will be addressed during the course, based on the author’s personal experience and on specific cases.
