143. évfolyam (2006) Supplementum I. 35 Diagnostic ability of GDx-VCC in discriminating normal from glaucomatous eyes in patients with early functional visual loss Giuseppe Di Stefano, Stefano Da Pozzo, Marco Paoloni, Giuseppe Ravalico University of Trieste (Italy), Eye Clinic Aim: to evaluate the ability of scanning laser polarimetry with variable corneal compensation (GDx-VCC) to distinguish healthy from glaucomatous eyes in patients with early visual field (VF) loss. Methods: 62 healthy subjects and 48 age-matched patients with early glaucoma (mean MD: -1.74±1.69dB) underwent complete ophthalmological evaluation, automated achromatic perimetry (AAP) and RNFL measurement with GDx-VCC. One randomly-selected eye from each subject was considered. AAP defects were defined as either the Glaucoma Hemifield Test (GHT) being outside normal limits or pattern standard deviation (PSD) being outside 95% confidence limits. Mean (±SD) MD, PSD and GDx-VCC parameters in the 2 groups of subjects were compared using the t-test. For each GDx-VCC parameter, the area under the ROC curve (AUROC) and the sensitivity at specificities =80% and =95% were calculated. Then, the parameter with the largest AUROC was evaluated using likelihood ratios (LRs). Results: Mean values for MD, PSD and for 10 of the 14 GDx-VCC parameters were significantly different between the 2 groups (p<0.001). The parameter with largest AUROC was NFI (0.870) and it displayed sensitivities of 80.2% and 60.4% respectively for specificities =80% and =95%. Interval LRs showed a large effect on post-test probability for NFI values =18 or =31. Conclusions: The GDx-VCC test showed moderate-to-good discrimination ability. The NFI parameter had the largest AUROC, but a considerable number of glaucomatous eyes (21 of 48, 43.8%) had NFI <30. This suggests that the algorithm for NFI calculation requires some refinement when eyes with early VF loss are evaluated. We present a case of an 8-year-old girl in whom a surgical procedure was performed due to symptomatic distichiasis and evidence of corneal irritation. Treatment involved lamellar eyelid splitting and cryotherapy of the posterior lamella.
