59. The properly signed and completed application shall be sent to the President. The application shall be approved from a medical standpoint in accordance with the standards and practices then currently applied by the Federation. Applicants approved by the President and Secretary- Treasurer shall be received into membership in accordance with existing By-Laws, and such membership shall be effec­tive the first day of the month when the applicant’s name is enrolled on the register of the Feder-ation. 60. The member may pay his dues in advance. For payments of dues for a period in excess of one year in advance, discounts will be applied at rates to be determined by the Secretary- Treasurer. 61. The monthly dues are due and payable to the Flome Office on the first day of each month and must be paid on or before the thirtieth (30th) day following said date. The member who fails to pay his dues by the end of the thirty (30) day period shall be suspended from membership rights by the office of the Secretary-Treasurer on the thirty-first (31 st) day after the due date. The limit of a member’s suspension shall be a maximum of three (3) months. If during this time, he pays all his arrearages, he shall be reinstated to membership. The suspended member shall inform the Branch Manager which form of withdrawal equity he desires. The Branch Manager shall report the member’s desire to the Home Office. Should the member make no choice, he automati­cally shall be placed on extended insurance. 62. A member who desires to change his membership from one branch to another shall make such request either to his own branch or to the Home Office. Upon his request, a transfer form shall be provided. The transfer shall become official when the properly completed and signed transfer form is ap­proved by the Secretary-Treasurer and recorded in the Home Office. A member shall belong either to the branch nearest to his residence, or to the Home Office branch. A member with more than one certificate shall have membership in only one branch in which all his certificates must be handled. ARTICLE IX BENEFICIARIES Eligibility - Estate - Changes - Lost Certificates 63. Each member of the Federation and holder of a beneficiary certificate shall, in the application for such beneficiary certificate, designate by name the person(s) to whom the benefits shall be paid. Said member shall have complete freedom in the selection of his beneficiary, except in those States where this is limited by law. 64. (a) In the event the designated beneficiary or beneficiaries die, or shall be for any reason disqualified, and no further designation is made by the member during his lifetime, the benefits so provided shall be payable to the estate of the member. (b) In the event that the member leaves no formal estate and/or designated personal representative of his estate, the benefits shall be payable to the member’s surviving spouse. If there is no surviving spouse, the benefits shall be payable equally to the member’s surviving children (including children by adoption). Children (including children by adoption) of the member’s deceased children (if any) shall take equally their deceased parent’s share. If there are no surviving children or grandchildren of the member, the benefits shall be payable equally to the member’s surviving parents or parent. If there is no surviving parent of the member, the benefits will be payable equally to the member’s surviving brother and sister. (c) Nothing contained herein shall be deemed to preclude the payment of benefits under this Section to a licensed funeral director or home which provides funeral services on behalf of the member, upon receipt of such request and documenta­tion as the Executive Committee shall, from time to time, deem appropriate. 65. A member is entitled to change his beneficiary upon reporting his request to the Branch Manager or the Home Office, sending the same, with his original certificate, to the Secretary-Treasurer. The change of beneficiary shall be effective upon signing of such request. 66. The beneficiary has no vested rights. When the member de­siring to change his beneficiary has been legally declared incompetent and a certified copy of such judgment or decree has been furnished to the office of the Secretary-Treasurer, such member cannot ask for a change of beneficiary. 67. In case a member has lost his certificate or if a certificate comes into the possession of any unauthorized person who refuses to part with it, the member desiring to change his beneficiary must attach to his request for this change, an af­fidavit signed in the presence of a Notary Public annulling his previous certificate and requesting a duplicate certifi­cate containing the name of this beneficiary. ARTICLE X DISCIPLINE OF MEMBERS Aim - Offenses - Procedure - Appeals 68. The aim of discipline is correction, not punishment. Sanctions should not be overly severe and expulsion from membership may be invoked only in cases of serious and repeated violations of the By-Laws. With reference to the beneficiary, the certificate holder may keep his certificate in force but shall not exercise any rights of elective or voting privileges. 69. Disciplinary proceedings may be instituted upon the follow­ing grounds: a) Refusal to comply with directions from higher author­ity. b) Actions in contradiction to the By-Laws. c) Agitation, in any form, against the Federation. d) Behavior contrary to law or good morals. e) Failure to perform duties in the best interests of the Federation.
