104. The monthly rates for juvenile insurance certificates are due and payable on the first day of each month. Should any payor fail or neglect to pay such rates within a period of 30 days from the due date, thereof the juvenile shall be suspended from membership. The certificate of the suspended member may be reinstated at any time within three months from the date of suspension by the payment of arrearages. 105. If the applicant named in the juvenile cer­tificate shall die, the certificate may be continued in force on the life of the insured member by any person legally responsible for his support and maintenance, and who shall assume payment of the required month­ly dues. ARTICLE XIV SICK BENEFIT CERTIFICATES Membership — Conditions — Dues Suspension — Cessation — Amount — Restrictions 106. The Federation provides for the payment of sick benefits to adult members and maintains a Sick Benefit Department for these purposes in com­pliance with the requirements of the laws of the states and provinces. 107. Membership in the Sick Benefit Department is not compulsory. Any adult member between the ages of 16 and 60 may apply for membership. If a member applies for sick benefit insurance simultaneously with his life insurance application, the same medical examination will be acceptable. Members on extended insurance are not eligible for membership in the Sick Benefit Department. Membership in the Sick Benefit Department granted after January 1, 1945, will terminate at age 60, except for the Income Protection Plan, which may be extended to age 65 at higher rates. 108. Members having paid-up life certificates may continue their membership in the sick Benefit Department if they pay 35 cents each month or $3.00 per year in advance over their regular month­ly sick benefit dues. 109. Application for a Sick Benefit Certificate shall be made upon the form of application blank prescribed by the Federation. The term of member­ship shall date from the day when the Secretary shall record the name of the accepted applicant in the official register of the Federation, together with the receipt of at least one month’s dues. 110. Women members holding certificates issued before 1960 shall be entitled to receive sick benefit of only $5.00 per week. Income Protection Plan is is­sued only to men, ages 16 - 55. 111. The monthly dues for sick benefit insur­ance are due and payable on the first day of each month, and if not paid within the grace period, the members shall be suspended from all benefits. A sus­pended member may be reinstated within three months upon the payment of all monthly dues. 112. No sick benefit shall be paid to a mem­ber: a) unless regularly attended by a physi­cian : M. D.; or osteopath; b) unless sickness is reported within 30 days, or within a reasonable period of time from the beginning of the disability. c) who within 6 months after his admission of membership is afflicted with cancer, nervous or mental disease, alcoholism or any other illness which was contracted prior to the date of ap­plication. d) on a certificate issued by a chiropractor, neuropath, dentist or midwife. The Federation reserves the right to reject any medical certificate and order a re-examination by another physician (M. D.) at the expense of the Federation. ARTICLE XV HOSPITALIZATION CERTIFICATES 113. The Federation issues individual hospitali­zation insurance certificates to adult members be- between the ages of 16 and 55. The Federation also issues family hospitalization insurance certificates to members in said age group which includes the spouse and children. 114. No medical examination shall be required for hospitalization insurance up to the age of 35. 115. Both individual and family hospitalization certificates are guaranteed renewable up to age 65. 116. The Federation shall provide for benefits specified in each certificate. Such benefits shall be payable directly to the member or his designated beneficiary and shall not be assignable. Hospitaliza­tion benefits shall be paid on submitted and certified claims and bills. Documentary proofs and bills to be submitted shall be the responsibility of the member. 117.
