Szemészet Supplementum I. Agnes Takács: Comparative analysis of corneal endothelial cell density after phacoemulsification performed by femtolaser-assisted and conventional cataract surgery. Purpose: To show the difference between femtolaser-assisted (LenSx-Alcon) and conventional phacoemulsification cataract surgery, based on the experiences with the first 300 cataract procedures carried out in the Department of Ophthalmology, Semmelweis University, Budapest, Hungary. Patients and methods: Anterior capsulotomy, corneal incision and femtolaser-assisted cross pattern and cylindrical pattern phacoemulsification cataract surgery was performed with an intraocular femtosecond laser (LenSx Lasers Inc. Irvine, CA, USA) in 40 studied eyes. As a control group conventional phacoemulsification cataract surgery was performed and analysed. During the postoperative follow-up digital retroilluminated pictures and Scheimpflug images were taken and evaluated. Wavefront aberrometry, corneal topography and objective visual quality were measured using the Nidek OPD Scan. Macular changes and corneal incision size and shape were measured by optical coherence tomography. Corneal endothelial cell density was analyzed by specular microscopy. Results: During the course it will be shown that in the femtolaser group a statistically significant difference was found in round shape, in height of the rhexis, the smallest and largest distance between the capsulotomy edge and the lens optics edge. Scheimpflug image analyses showed significantly less vertical tilt of IOLs implanted after femtolaser-assisted capsulotomy. The LenSx group had significantly lower values of intraocular vertical tilt, coma and a significant increase in Strehl ratios. Femtolaser assisted cataract extraction was associated with less endothelial cell loss and a lower rate of cystoid macular edema. Conclusion: Femtosecond laser assisted capsulorhexis, corneal incision and nuclear fragmentation is a safe and useful method in cataract surgery. Kurzus
