147. évfolyam (2010) Supplementum I. Is there a correlation between functional and anatomical improvement after intravitreal anti- VEGF therapy in AMD patients? Katalin Tóth-Kovács, Zsuzsanna Pamer, Zsolt Bíró University of Pécs, Pécs, Ophthalmology Clinic Purpose: To evaluate the therapeutical effects of intravitreal ranibizumab injections in patients suffering from the wet form of age-related macular degeneration (AMD). Methods: 115 eyes of 111 AMD patients with subfoveal neovascularization were treated with intravitreal injections of ranibizumab (0.5 mg) in 3 consecutive months. Retreatment was indicated when more than 5 ETDRS-letters visual acuity (VA) loss occurred, or a significant increase in the retinal thickness was detected by Optical Coherence Tomography (OCT) during the monthly follow-ups. The best improvement in VA, the VA at the last visit, and the corresponding OCT images were evaluated. Results: The mean follow-up was 11.4±6.4 months. The best mean VA improvement was 11.0 letters (pcO.OOl). 78% of the eyes improved more than 5, and 30% improved more than 15 letters. The mean VA gain at the last visit was 6.3 letters (pcO.OOl). At the last visit an improvement of more than 5 letters occurred in 57% and more than 15 letters occurred in 24% of the eyes. The best VA outcomes were achieved with an average of 3.6 injections, and the average number of injections per patient during the follow-up period was 4.6. The average central retinal thickness on OCT decreased from 341.7 to 253.8 microns until the best VA improvement, and to 263.1 microns until the last visit (p<0.001). There was no significant correlation between OCT measurements and visual gain (r=0.17). Conclusions: In agreement with international studies our results show that intravitreal anti-VEGF injections improve visual functions in wet-AMD involving the fovea more effectively than any previous treatment strategies. Beside VA and OCT, fundus examination is also mandatory to disclose scarring or atrophy.
