147. évfolyam (2010) Supplementum L 81 Iridocyclectomy in the treatment of anterior segment tumours Jenő Tóth Saint George Hospital of Fejér County, Székesfehérvár, Ophthalmology Department Due to their position ciliary body tumours are rarely recognised at an early stage. They may be detected by screening tests or by examinations based on other casual symptoms only. Patients and method: Processing the materials from the past 20 years, tumours were found in 17 cases; 12 of these also affected the iris and in 5 cases they were isolated in the ciliary body (8 females, 9 males). When iridocyclectomies were performed, the patients’ average age was 54.6 years. Because of the size of the tumour enucleation was the only solution in 11 cases. In one of the cases ruthenium irradiation was applied. If the size of the tumour did not exceed one quadrant of the iris, did not reach the ora serrata, did not show either signs of scleral infiltration or secondary glaucoma then bv the application of ITN and forced systolic hypotonia - due to the risk of haemorrhage - iridocyclectomy was performed with a bisected scleral flap (1 female, 4 male patients). Results: We found 2 spindle cell iris-ciliary body melanomas, 1 mixed cell iris-ciliary body melanoma, 1 isolated ciliary body spindle cell melanoma, 1 pigmented ciliary body Schwannoma. During the postoperative phase, an absorbing vitreous body bleeding was detected in 4 of the 5 cases. Following the absorption of the haemorrhage, vision was between 0.2 and 0.6. Cataract developed in 3 of the 5 patients after the iridocyclectomy, so one ECCE+PCL surgery and two phaco + PCL surgeries were done. During the average follow-up period of 13.2 years, no relapse occurred. In one of the cases AION explains the vision of 0.02, while in the other case the corneal decompensation occurred 7 years after the ECCE surgery. YAG laser cap­­sulotomy was necessary in one case. In summary: In line with the recommendations in medical literature, in the case of smaller iris-ciliary body tumours, iridocyclectomy with proper preparation may result not only in retaining the globe, but also useful vision- and a relapse-free status may also be achieved.
