period the patient developed multiple infiltrates. Corneal scrapes revealed fungal keratitis. He received intensive treatment with topical voriconazole 1% and amphotericin B, and systemic voriconazole 200mg 2x1. After one week of treatment the corneal infiltration decreased and the patient had no more symptoms.

Case 2: A 46-year old patient with a history of corneal injury presented with initial treatment of gentamicin/dexamethason and acyclovir ointment. The cornea showed a central disciform opacification. After corneal scraping and diagnosis of fungal keratitis the original treatment was stopped and local and systemic voriconazole was initiated. The keratitis healed two weeks after onset of treatment.

Conclusions: The incidence of fungal keratitis is very low in our continent, but it should be suspected in patients with history of trauma, foreign-body contact or wound contamination by plant material. Voriconazole as a broadspectrum antifungal agent can be recommended for initial treatment of fungal keratitis.

Teleophthalmology - past and future

18 years after the first direct, synchronous connection between ophthalmologists over long distances, it is time for a critical retrospective assessment concerning the enduring results, and also the potential future prospects for telematics in ophthalmology. In these 18 years great personal, scientific, technological, and administrative efforts have been made. Large projects have been supported by local governments and by the European Union. MEDKOM, OPHTEL, Bavaria/Slovenia and TOSCA have in due course succeeded in involving several European countries. Synchronous and asynchronous techniques have been developed, and also various different technologies of high-speed, secure data transfer (e.g. via fibre-optic links, satellite, and ISDN). From the start, not only the exchange of expert knowledge has been important, but the personal integration of the patient as well. From this aspect, I can foresee future prospects for all the trials. At the present time, all hospital projects of mere scientific interest will definitely fall under the economic axe. But patient-oriented projects not only address genuine human needs, but might also more easily survive financial restrictions; and, possibly, reduce health-care costs.

A corneális endothelium vizsgálata újfejlesztésű non-kontakt spekulár mikroszkópokkal

Célkitűzés: A corneális endothelium vizsgálata non-kontakt spekulár mikroszkópokkal normális corneákon.

Betegek és módszerek: A méréseket 62 egészséges egyén (átlagéletkor: 48,2±29,1 év) - statisztikai értékelést javítandó - jobb szemén végeztük el. Ezeknek a személyeknek szemészeti betegsége sem korábban, sem jelenleg nem volt, általános betegségben nem szenvedtek. Szférikus és cilindrikus törési hibájuk nem haladta meg a ±3,0 dioptriát. Egy corneán 3-3 mérést végeztünk egy új fejlesztésű, PC vezérelt, saját szoftverrel ellátott non-kontakt (Sea Eagle, Rhine Tec, Krefeld, Németország) valamint egy automata, szintén non-kontakt elven működő corneális endothel-mikroszkóppal (NonCon Robo, Konan, Tokyo, Japán). A mindkét készülékbe beépített képelemző rendszerrel meghatároztuk a sejtsűrűséget és a hexagonális sejtek arányát. A kapott adatokat az átlag±SD-vel jellemeztük. A változókat Wilcoxon teszttel hasonlítottuk össze. A mérések reprodukálhatóságát a Cronbach szerinti a-koefficienssel jellemeztük.

Eredmények: Az átlagos endotheliális sejtsűrűség 2604±602 sejt/mm2 volt a Sea Eagle és 2423±636 sejt/mm2 volt a NonCon Robo műszerrel (p<0,0001, Wilcoxon). A hexagonalitás értéke 39,19±6-nak illetve 57,17±10-nek bizonyult a készülékek fenti sorrendjét követve (p<0,0001). Ä sejtsűrűség esetében az a-koefficiens 0,98 illetve 0,97 volt, szintén a műszerek fenti sorrendje szerint. A hexagonalitás esetében az а-koefficiens a Sea Eagle-nél 0,61-nek, a NonCon Robonál 0,49-nek adódott.
