Diabetic vitrectomy in a general hospital Milibák, Tibor Dept, of Ophthalmology, Uzsoki Teaching Hospital, Budapest Diabetic vitrectomy is the most frequent vitreoretinal intervetion. Advances in our understanding of the pathophysiologic mechanisms of diabetic retinopathy, and improvements in instrumentation and surgical techniques, have significantly expanded the indications for diabetic vitrectomy in recent years. The question arises as to whether a general vitreoretinal practice is capable of keeping up with this rapid improvement. According to our experience, under favourable conditions the answer is yes. The most important factor is the skill of the vitreoretinal surgeon. Instrumentation however, which is also necessary to effective surgery, can only be obtained gradually. Based on the skills of the surgeon and the level of available instrumentation, careful selection of patients is inevitable in order to minimise unnecessary complications. fi::; 113 Diabeteses járóbetegek szemészeti szakgondozása Tornai Ildikó, Molnár Szilvia, Sallai Tímea Semmelweis Kórház, Szemészeti Osztály, Miskolc 2001-ben 539 diabteses retinopathiás járóbeteget vizsgáltunk meg egy vagy több alkalommal. 104 beteget a háziorvos utalt be az osztályra, míg 16-ot a diabetológiai szakrendelés, 419-et pedig a szemészeti szakrendelés A diabeteses retinopathia fennállásának és súlyosságának vizsgálatát pupillatágításban réslámpával (fundus- vagy kontaktlencsével) és FLAG-gal végeztük. A diabeteses retinopathiaként diagnosztizált esetek lézerkezelést igényeltek (304 eset), ha egyik vagy mindkét szemen klinikailag szignifikáns macula-oedema vagy proliferativ diabeteses retionpathia volt. Lézerkezelést 1-5 alkalommal végeztünk 304 szemen. Ezután a betegeket 4-12 hetenként ellenőriztük. 15 betegnél javasoltunk vitrectomiát, ami 11 esetben megtörtént: néhányat osztályunkon, a többit más szemészeti osztályon végezték el. Ophthalmologic screening of diabetic outpatients Tornai, Ildikó, Molnár, Szilvia, Sallai, Tímea Dept, of Ophthalmology, Semmelweis Hospital, Miskolc In our hospital 539 outpatients with diabetic retinopathy were examined one or more times during 2001. 104 patients were direct to the Department by the family doctor, while the other patients were send for examination by the Diabetic Care Unit (16 patients) or the Ophthalmologic Health Centre (419 patients). Fundi were examined for presence and grade of diabetic retinopathy by slit lamp biomicroscopy (with fundus and contact lenses) after pupil dilatation and fluorescent angiography. The events were defined as diabetic retinopathy requiring laser treatment (304 cases), if there was either clinically significant diabetic macular oedema (CSMO) or proliferative diabetic retinopathy (PDR) in one or both eyes. In 304 eyes laser treatment was carried out 1-5 times. Follow-up is after an interval of 4-12 weeks. 15 patients were recommended for vitrectomy and in 11 cases it was performed; some of these were done in our department, and the remainder in other eye departments. 31 Augustus 2002
