A korábbi klasszifikációs rendszerrel szemben az új rendszer a nem plakk okozta csoportba, a speciális bakteriális fertőzések közé sorolta be a necrotizáló parodontális betegségeket is. Korábban ez csak a fogágybetegségek kategóriájában szerepelt. Bár mint látni fogjuk, a rögzítő apparátus betegségei csoportban is felsorolja a rendszer a necrotizáló parodontitist. Tény, hogy a NP-nak specifikus bakteriális flórája van és etiológiájában jól körülhatárolható mikroorganizmusok szerepelnek. Ezek egy része tagja a paradontopatogén baktériumok nagy családjának (Fusobacterium spp.,

and Prevotella intermedia). Azonban a NP valódi fertőzés, mert házastársak között átvihető, valamint teljesen ép parodontiumon, dentális plakk hiányában is kialakulhat, elsősorban stressz hatására.
