These immediate dentures were worn 12-24 weeks by our patients. The implants were removed during the preparation of the definitive dental prosthesis. In the examined period we have lost 2 implants. 2 further implants were to be removed 8 weeks after insertion possi­bly due to a failure of the immediate denture. No inflammation or pain around the ORTHOPLANT® im­plants has been registered during the observation period. KIVOVICS Semmelweis University, Faculty of Dentistry, Department of Prosthodontics, Budapest THE CONSTRUCTING OF THE NEW DENTAL ARCH IN THE PROSTHETIC REHABILITATION OF THE CLEFT LIP AND PALATE PATIENT Multidisciplinary team deals with cleft lip and palate patients in accordance with a preprojected, complex treatment plan­ning. At the infant years the treatment is started according to this deformity. The definitive prosthetic care for cleft patients is usually indicated as the last phase of the process of treat­ment to correct the palatal defect, the malocclusion and the missing teeth. The additional goals of prosthetic rehabilitation for these patients must include adequate articulation and acceptable aesthetic appearance. First of all because of the last requirement we should take severe biomechanical di­rectives into consideration. The maintainces of the biome­chanical rules, the aesthetic appearance and oral hygiene with prostheses are often difficult because of scars in the vestibule of the oral cavity and stiffness of the upper lip. The authors emphasise the preliminary steps by using diagnostic casts and try-in dentures to analise the single cases easier. The authors describe the prosthodontic rehabilitation of an adult patient with unilateral cleft lip and palate. This clinical report presents the prosthodontic treatment with “new-arch” pros­thesis made of titanium alloy. The patient has already wore her new dentures for more than one year without any problem. She is satisfied with its aestetic appearance, the function of the masticatory apparatus and speech. The authors also emphasise the importance of the regular control. RADNAI University of Szeged, Faculty of Medicine, Department of Dentistry and Oral Surgery, Szeged; 1 University of Szeged, Faculty of Medicine, Department of Otorhinolaryngology and Head & Neck Surgery, Szeged THE INFLUENCE OF WEARING COMPLETE DENTURE ON PHONETICS The oral cavity and its anatomical structures, both on their own and in their relative positions to each other, play an important role as a sound source and modifier. This is true in the case of patients who wear fixed dentures or removable partial or complete dentures, as well as with natural dentition. A person’s speech capability changes as the number of remaining teeth decreases. In the case of total edentulousness the production of vowels and consonants is impaired due to the change in the shape and volume of the oral cavity, as the ratio of the soft and the hard tissues, the position and form of the lips and tongue, as well as the structure and size of the alveolar ridge alter. Of course there is an adaptation process to the changing situation. The aim of the study was to estimate the influence of the full denture on the articulation of the patients. Patients wearing complete dentures were examined. Pa­tients were asked to utter specific words both with and without their dentures. Speed, clarity of speech and changes in “hiss­ing” sounds were noted. The changes in the pronunciation of vowels and consonants were analysed with the help of a computer program, which provided an objective assess­ment. Surgery, Szentes IMMEDIATE LOADING OF IMPLANTS ON THE BASIS OF LITERATURE AND LONG TIME EXPERIENCE WITH DENTI® IMPLANT INSERTIONS Immediate loading of dental implants have been widely used to retain and support cross-arch partial dentures. 3-6 months after implantations the osseointegration is established. This period may be shortened with immediate loading of implants. The most valuable and relevant articles were selected from literature, especially from Medline search. The influenc­ing factors of success of immediate loading could be divided into the following four categories: surgery-, host-, implant-, and occlusion-related factors. The article of Gapski helped us to define the evaluatuion factors of success and criteria (Clin Oral Impl Res 2003, 14) too. The immediate loading of DenTi® implants began 12 years ago. During this period the clinical results have been inves­tigated in case of 73 patients and of 190 DenTi® implants. The control was carried out with 263 implants placed with submerged technique of the same patients.
