We have to distinguish between the diseases, in which the causative part of a foregoing focal infection is evident, such as tonsillitis and consecutive rheumatic fever or nephritis, and other diseases, in which neither the anamnesis, nor the clinical examination gives a sure argu­ment in favour of the alleged focal origin. In the former diseases, the tonsils serve perhaps only as entrance of the infection and offer later no pathological changes; in the laitter diseases, chronic inflammation of the tonsils, dental granuloma etc. are supposed to support the diseases in Question- In the first period of the theory of focal infection it was supposed, that streptococci of the foci incidentally enter the blood stream and cause metastatic inflammations in the various organs. The uniformity of the morbid processi was ezplained, by Rosenow, through the theory of elective localization. Control ezperiments resulted in the confirmation of the existence of strongly artrophii strains of strepto­cocci, but the existence of an elective localization could not be confir­med. Whatever the modus of infection of the experimental animals may nave-been, the pathological changes were of septic nature and did not resemble the histological changes of human rheumatism or nephritis. The inefficacity of nodern chemotherapy in these conditions is an argument more against the infectious theory. In the last decade, foci have been considered rather ais sources of sesmbilization with antigens of various origin and the secondary diseases ás hyperergic reaction against these antigens. Specific antibody response has however not been proved in the diseases in question and it is not certain whether experi­mental hyperergic tissue response is identical with the histological changes seen in human diseases. The lack of eosinophil reaction is a striking point too. In the absence of experimental confirmation, pre­caution is indicated relating to surgical treatment of alleged foci: the latters should exhibit signs of probable activity. Only in the acute stage of diseases of inflammatory character should surgical treatment of foci be considered and only, when hematological or other signs of active infection are still present. A fogorvos praxisai folya­mán megszokik egy bizonyos munkame­netét és nem is gondol arra, hogy azon időnkélnt változtatni lehetniei, sőt kellene.
