Section 303.1 He shall be residing in the United States of America, its territories or possessions or any country on the North American continent. 303.2 He is under the regular care of a Licensed Prac­titioner and he certifies period of total disability on claim form. 303.3 If a member is treated in a hospital, a copy of the hospital records will be accepted by the Associa­tion in lieu of the claim form furnished by the Asso­ciation. 304 Sick benefits shall not be paid in the following cases: 304.1 For partial disability. 304.2 Those who willfully injure themselves. 304.3 Those who are in arrears with more than three monthly dues. 304.4 Those who do not report and file a claim form within two years from onset of injury or sickness. 305 Life insurance members who have lapsed their membership certificates, or who have changed same to a Reduced Paid-Up Life, or who are in Extended Insurance, shall automatically cease to be members of the sick benefit class. 306 Members whose life insurance certificates become fully paid-up, or become matured endowments, may remain members of the sick benefit class by continuing to pay the prescribed monthly dues. 401 All sick benefit and maiming benefit claims shall be paid from the sick benefit fund of the Association. 402 Sick benefit claim forms must be submitted to the Branch Manager, who in turn must properly complete his portion of same and remit it to the Home Office immediately . 403 Benefits due a member who has deceased shall be paid by the Association to the designated beneficiary or Estate. 404 If a member is incompetent, benefits due him shall be paid to the spouse; if none, to the parents; if none to the children. If the member shall have no relatives, the benefits due him shall be paid to the legally ap­pointed conservator or guardian. MAIMING BENEFITS 501 Anyone who is a member of any of the sick benefit classes and who shall lose at the same time: 501.1 The entire thumb, or the thumb and one finger, or three fingers of one hand, shall receive $100.00. (The loss of a joint of a finger shall mean a loss of such finger.) Section 502 $200.00 Maiming Benefit is payable to the member who shall lose: 502.1 One eye. (The loss of eyesight is equivalent in meaning with the loss of an eye diseases.) or, 502.2 One hand, or 502.3 One foot. (The loss of the metatarsus or heel shall be considered the loss of the foot.) 503 $400.00 Maiming Benefit is payable to the member who shall lose at the same time: 503.1 Both eyes, or 503.2 Both hands, or 503.3 Both feet, or 503.4 One eye and one hand, or one eye and one foot, or one foot, or 503.5 One hand and one foot. 504 Total and permanent paralysis of the above named members of the body shall be construed to be a total loss of the said part of the body regardless of the cause of such paralysis. Fracture of the spine result­ing in total paralysis of the back, causing total and permanent disability, shall be considered as being totally maimed. 505 A member shall be entitled to receive a maximum of $400.00 maiming benefits on each certificate to which there is attached a sick benefit rider. 506 A member who has received the maximum amount of sick benefit due him, may retain his rights to maiming benefits at no cost, provided such members have not already received the maximum amount of maiming benefits. 507 Members who have withdrawn 90% of sick benefits they are entitled to, may request dropping sick benefit coverage with the provision that maiming benefit be kept in force at no cost. 508 Former Verhovay members, who joined the sick benefit class after January 1, 1944 shall be covered for maiming benefit to the anniversary date nearest the member’s attained age of 65. Members who joined prior to January 1, 1944 shall have maiming benefit coverage renewable for life, or until all benefits have been consumed. 509 Former Rákóczi members, who joined after January 1, 1940, belonging to Sick Benefit Class II, shall be covered for maiming benefit until their 65th birthday. Those in Class I, who joined prior to January 1, 1940, shall have maiming benefit coverage renewable for life, or until all benefits have been received. 13
