Face amount payable to member on certificate anniversary nearest to his 85th birth­day, or, upon prior death, to beneficiaries. — Monthly dues per $1,000 insur­ance: $1.65 at age 20, — $2.08 at age 30, — $2.81 at age 40, etc. — Per $2,000 insurance: $2.91 at age 20, — $3.77 at age 30, — $5.23 at age 40, etc., (plus 5 cents per certificate toward branch administration.) imum benefit period during one calendar year 20 weeks. Dues pay­able for 20 years, or until prior death of member. Face amount payable to beneficiaries immediately upon the death of the member. — Monthly dues per $1,000 insurance: $2.63 at age 20, — $3.09 at age 30, — $3.77 at age 40, etc. — Per $2,000 insurance: $4.87 at age 20, — $5.79 at age 30, — $7.14 at age 40, etc., (plus 5 cents as above.) CLASS “C” — 20 YEAR ENDOWMENT INSURANCE Combination of guaranteed savings plan and protection. Dues payable for 20 years, or until prior death. Face amount payable in full to member on 20th anniversary of certificate, or to beneficiaries upon prior death. — Monthly dues per $1,000 insurance: $4.33 at age 20, — $4.40 at age 30, — $4.65 at age 40. etc. — Per $2,000 insurance: $8.26 at age 20. — $8.41 at age 30, — $8.90 at age 40. etc., (plus 5 cents, as above.) Monthly dues per $1,000 insurance: $2.00 at birth, $2.10 at age 5. $2.26 at age 10, etc. Per $2.000 insurance: $3.61 at birth, $3.81 at age 5, $4.13 at age 10, etc., (plus 5 cents, as above). CLASS “D” — 30 YEAR ENDOWMENT INSURANCE Combination of guaranteed savings plan and protection. Especially suit­able for those desiring savings plan but unable to pay 20 Year Endowment rales. Dues payable for 30 years, or until prior death of member. Face amount payable in full to member on 30th anniversary of certificate, or to beneficiaries immediately upon death of member if occurring prior to maturity date. — Monthly dues at $1,000 insurance: $2.87 at age 20, — $2.96 at age 30, — $^,32 at age 40, etc. — Per $2.000 insurance: $5.34 at age 20, — $5.52 at age 30, — $6.24 at age 40, etc., (plus 5 cents, as above.) CLASS “E” — ENDOWMENT AT AGE 60 INSURANCE Combination of guaranteed savings plan and protection, designed par­ticularly to provide old age security. Dues payable until certificate anniver­sary nearest to member’s 60th birthday, or until prior death. Face amount payable to member on certificate anniversary nearest his 60th birthday, or to beneficiaries immediately upon prior death. — Monthly dues per $1,000 insur­ance: $2.14 at age 20, — $2,96 at age 30, — $4.65 at age 40, etc. — Per $2,000 insurance: $3.88 at age 20, -— $5.52 at age 30, — $8.90 at age 40, etc., (plus 5 cents as above.) Issued up to age 55. CLASS “F” — ENDOWMENT AT AGE 70 INSURANCE Combination of guaranteed savings plan and protection, designed to pro­vide security for high age, at low cost. Dues payable until certificate anni­versary nearest the member’s 70th birthday, or until prior death. Face amount payable to member on the certificate anniversary nearest to member’s 70th b'rthdav, or to beneficiaries immediately upon prior death of member. Monthly dues per $1,000 insurance: $1.80 at age 20, — $2.34 at age 30, — $3.32 at age 40, etc. — Per $2,000 insurance: $3.21 at age 20, — $4.28 at age 30, — $6.21 at age 40 etc., (plus 5 cents, as above.) CLASS “G” — TWENTY PAYMENT — i ENDOWMENT AND 1 PAID-UP LIFE INSURANCE Combination of 20 Year Endowment and 20 Payment Life insurance. Dues payable for 20 years. Half of face amount payable to member on 20th certificate anniversary, the other half payable to beneficiaries upon death of member. In the event of death prior to maturity date, entire face amount pay­able to beneficiaries immediately following death of member. — Monthly dues per $1,000 insurance: $3.43 at age 20, — $3.75 at age 30, — $4.21 at age 40. etc. — Per $2.000: $6.56 at age 20, — $7.10 at age 30, — $8.02 at age 40, etc., (plus 5 cents, as above.) ACCIDENTAL DEATH BENEFIT INSURANCE Additional accidental death benefit insurance (Double indemnity) con­tracts up to $2,000. Contract expires at certificate anniversary nearest to member’s 70th birthday. Monthly dues 15 cents for $500, — 25 cents for $1,000, — 10 cents for $1,500, — and 50 cents for $2,000. SICK BENEFIT INSURANCE CLASS I Dues: $0.45 monthly, Benefits: $4.00 per week. Admission: age 16 to 55, men, age 16 to 40, women. CLASS III Dues: $1.40 per month Benefits: $12.00 per week Admission: age 16 to 40, men only. CLASS II Dues: $0.90 per month. Benefits: $8.00 per week, Admission: age 16-45, men, age 16 to 40, women. CLASS IV Dues: $1.90 per m^nth Benefits: $16.00 per week Admission- age 16 to 35, men only. Contract expires at member’s 65th birthday, and, also, upon entry in mi­litary service. Benefits payable beginning as of 8th day following report of disability to manager. No benefits paid for first 7 days. Maximum coverage 62] weeks, excepting continuous illnesses limiting coverage to 50 weeks. Max­( LASS “C-J” — TWENTY YEAR ENDOWMENT INSURANCE Combination of guaranteed savings plan and protection, dues payable for 20 years. Face amount payable to member on 20th anniversary of certificate, or to beneficiaries upon prior death. (See schedule of benefits beloiw). Monthly dues per $1,000 insurance: $1.23 at birth, $4.25 at age 5, $4.27 at age 10, etc. For $2,009 insurance: $3.06 at birth, $8.10 at age 5, $8.14 at age 10, etc., (plus 5 cents, as above.) CLASS “H-J” — ENDOWMENT AT AGE 18 INSURANCE Comb'naUon of guaranteed savings plan plus protection, designed es­pecially to provide funds for higher education of member. Dues payable until certificate anniversary nearest to member’s 18th birthday, or until prior death. Face amount payable to member on certificate anniversary nearest to 18th birthday, or to beneficiaries upon prior death. (See schedule of benefits below.) 'Monthly dues per SI,000 insurance: $4.73 at birth, $5.05 at age 1, $6.73 at age 5, $8.89 at age 8, $11.25 at age 10, etc. Per $2,000 insurance: $9.07 at birth, $13.06 at age 5, $22.10 at age 10, etc., (pius 5 cents, as above). Issued up to nge 15. SCHEDULE OF BENEFITS The rates quoted above apply only in stales permitting the writing of cer­tificates with “Full Benefit At Age 5”. This excludes the states of New York, Connecticut and Virginia, where statutes prohibit the payment of full death benefits in cases of death occurring prior to age 10. Rites on certificates»pro­viding payment of “Full Benefit At Age 10’’, (N.Y., Conn., Va.) are slightly lower. Death benefits payable in states permitting the payment of full benefit at age 5, are — per $1,000 insurance — as follows: $100 within 6 months after date of birth, $200 at age 1, $400 at age 2, $600 at age 3, $800 at age 4, $1,000 at age 5 and after. PAYOR BENEFITS TO AGE 21 OR PRIOR MATURITY ' , Payor insurance providing, in the event of death of payor, waiver of dues on Juvenile contract until certificate anniversary nearest 21st birthday of (juvenile) member, or until prior maturity. Payor may be parent, grand­parent or guard'an. Medical proof of insurability of payor required. Available in all juvenile classes. Monthly dues (to be added to regular dues quoted above) per $1,000 insurance, age of payor 25, age pf juvenile member 1: 10 cents in Class “AA-J” — 14 cents in Class “B-J” — 23 cents in Class “C-J” — 22 cents in ('lass “H-J”. ACCIDENTAL DEATH BENEFIT INSURANCE Accidental death benefit insurance (double indemnity) available to juvenile members of age 10 or older. Conditions and rates same as in Senior Order. CLASS “C” TERM JUVENILE INSURANCE Insurance protection at lowest possible cost from birth to 16th birthday when certificate expires. No cash or loan values. Approximately 30% of net mortuary dues refunded if exchanged for Ordinary certificates after member­ship in good standing for three years. Term contracts provide for death and maiming benefits. Full death benefit $500. — Dues 40 cents per month. NON-FORFEITURE VALUES Extended, Pakl-up Insurance, Cash Surrender and Loan values available on all Ordinary insurance certificates (Class “C” Term Juvenile excepted) after 3 years' membership in good standing.
