Page 12 TESTVÉRISÉG AN IMPORTANT NOTICE REGARDING HRFA’s CONSUMER PRIVACY Recent Federal legislation known as the Financial Services Modern­ization Act (also known as the Gramm-Leach-Bliley Act) has im­posed a great number of changes to the financial services industry. In connection with these changes, financial institutions, like the FIRFA, are required to establish privacy policies and procedures. They are also required to tell their customers how personal information that the financial institution maintains about them is treated. This ex­plains the many notices that you have been receiving lately from banks, credit card companies and other financial institutions with which you may transact business. Information We Collect Most of the personal information that we collect from you is pro­vided by you. In your application for life insurance, you provided us with certain personal information (name, address, date of birth, etc.) and also certain information regarding your health and medical his­tory. If you have ever applied for a mortgage loan from the HRFA, you provided us with certain financial information (income, employ­ments, assets, liabilities, etc.). The HRFA may have also obtained a credit report on you from a credit-reporting agency. Treatment of Information Generally, the information we receive from you is used internally to evaluate your applications for life insurance and mortgage loans, per­form general administrative activities and assist in processing trans­actions requested by you. On certain larger face amount applications for life insurance, this information may also be disclosed to our rein­surance company. There may be other instances when we may be required by law to make disclosure, for example, in response to a valid subpoena or court order. We may also make other disclosures of information as permitted by law. All nonpublic personal health and financial information is treated with the utmost confidentiality by our employees, and appropriate safeguards and procedures are in place to protect that confidentiality, including appropriate security mea­sures regarding computer access to such information. Sharing of Information The HRFA will never sell or disclose for profit any of your private personal information whatsoever to any third parties who may wish to solicit you regarding the purchase of other products or services. The HRFA will not disclose any private health, medical or financial information about you to any third parties except (1) to process a transaction requested by you, (2) to carry out a transaction in the normal course of our business or (3) as may be required by law. Since its inception in 1896, the Hungarian Reformed Federation of America has always been aware of the necessity of protecting the privacy of its members and applicants, and has always taken consci­entious action to achieve that goal. Privacy policies and procedures have always been in place and stringently followed by our employ­ees. These privacy and confidentiality policies and procedures have been set down in writing in our Employee Handbook and other memo­randa, and the employees have been advised and instructed accord­ingly as to these policies and procedures. The HRFA also recognizes its mandate as a fraternal association of Hungarian-Americans to promote and encourage fellowship and com­munication among its members and among people of Hungarian an­cestry here in America and throughout the world. The HRFA also seeks to stimulate and support the cultural and religious traditions and heritage of Hungarians in America. We believe that our members, by their affiliation with us and their participation in our fraternal activities, share and support these objectives. Accordingly, to further these objectives, the HRFA may provide certain information (for example, name, address, age group) about you to non-affiliated organizations in the Hungarian-American and global Hungarian communities that share these objectives, as well as to the Bethlen Home of the HRFA, an affiliated health care facility located in Ligonier, Pennsylvania. However, any such disclosure will be under the following conditions: (1) no dates of birth or Social Security numbers of any individual will be disclosed; (2) no informa­tion will be provided to any organization whose activities are basi­cally political in nature; and (3) the organization receiving the infor­mation must agree in writing not to make any further disclosure of the information with the consent of yourself or the HRFA. Also, you have the right to choose not to have this information disclosed to such organizations, as discussed in the following section. Your Choice Not to Participate At any time you may choose not to participate in the sharing of information with non-affiliated organizations as described in the pre­vious section. If you choose not to participate, you may complete the Opt-out-Reply Form appearing at the end of this notice and return it to the HRFA Home Office as indicated therein. You may also contact us in writing specifying your choice to Opt-out from the sharing of information, including your name, address, telephone num­ber, certificate number and signature. Maintaining this Policy The HRFA will review its Privacy Policy and practices from time to time based upon changing business conditions and any changes in our information collection or sharing procedures. The HRFA reserves the right to change or amend its Privacy Policy and practices at any time and our members will be notified in accordance with applicable laws and regulations.
