107. Members having paid-up life certificates may continue their membership in the sick Benefit Department if they pay 35 cents each month or $3.00 per year in advance over their regular monthly sick benefit dues. 108. Application for a Sick Benefit Certificate shall be made upon the form of application blank prescribed by the Federation. The term of membership shall date from the day when the Secretary shall record the name of the accepted applicant in the official register of the Federation, together with the receipt of at least one month’s dues. 109. Women members holding certificates issued before 1960 shall be entitled to receive sick benefit of only $5.00 per week. Income Protection Plan is issued only to men, ages 16-55. 110. The monthly dues for sick benefit insurance are due and payable on the first day of each month, and if not paid within the grace period, the members shall be suspended from all benefits. A suspended member may be reinstated within three months upon the payment of all monthly dues. 111. No sick benefit shall be paid to a member: a) unless regularly attended by a physician: M. D.; or osteopath; b) unless sickness is reported within 30 days, or within a reasonable period of time from the beginning of the disability. c) who within 6 months after his admission of membership is afflicted with cancer, nervous or mental disease, alcoholism or any other illness which was contracted prior to the date of application. d) on a certificate issued by a chiropractor, neuropath, dentist or midwife. The Federation reserves the right to reject any medical certificate and order a re-examination by another physician (M. D.) at the expense of the Federation. ARTICLE XV HOSPITALIZATION CERTIFICATES 112. The Federation issues individual hospitalization insurance certificates to adult members between the ages of 16 and 55. The Federation also issues family hospitalization insurance certificates to members in said age group which includes the spouse and children. 113. No medical examination shall be required for hospitalization insurance up to the age of 35. 114. Both individual and family hospitalization certificates are guaranteed renewable up to age 65. 115. The Federation shall provide for benefits specified in each certificate. Such benefits shall be payable directly to the member or his designated beneficiary and shall not be assignable. Hospitalization benefits shall be paid on submitted and certified claims and bills. Documentary proofs and bills to be submitted shall be the responsibility of the member. 116. While the Federation cannot cancel hospitalization certificates before the age of 65, it reserves the right to levy additional assessments. Rates will not be changed for individual members. Any rate change necessary will apply to all members of the same class. This stipulation will not apply to non-cancellable, guaranteed, renewable hospitalization certificates. 117. Hospitalization insurance certificates shall be issued in the form of a rider or as a separate certificate to the life insurance certificate of a member. ARTICLE XVI COVERAGE 118. All provisions of these by-laws shall be applicable with the same force and effect to all members and branches located in the various states of the United States and the various provinces of Canada. ARTICLE XVII MISCELLANEOUS PROVISIONS 119. Only the Executive Committee of the Federation shall have the power to waive any provisions of these by-laws relating to admissibility to membership, payment of dues and assessments, change of beneficiaries, or any of the provisions of the membership certificates; and any action taken by anyone else through inadvertence or oversight shall not operate by way of waiver or estoppel against the Federation. 120. Should any provision of these by-laws conflict with the laws of any of the states or provinces wherein the Federation is licensed to operate, the provisions of the laws of the states and provinces shall prevail to the same extent as if specifically incorporated herein. 121. Whenever the adult membership of the Federation shall diminish to one hundred fifty (150) members or less, a special meeting of the Convention shall he called, whereupon a plan of procedure shall be adopted as to the dissolution or reorganization of the Federation. 15
