ESZENYI, Auditor LOSS OF NATURALIZATION CERTIFICATE Question: I was naturalized as an American citizen many years ago. In connection with a passport application, I now need proof of my citizenship, but cannot find my certificate of naturalization. Is there any way in which it can be replaced? Answer: Yes, there is. Anyone whose original Certificate of Naturali­zation or Citizenship has been lost or mutilated or destroyed, or anyone whose name has been changed by order of a court or by marriage sub­sequent to the issuance of such certificate and who wishes a certificate in his new name, may make application for a new certificate on Form N-565. The fee is, ordinarily, $5.00. No fee is required if the applicant has served honorably in the armed forces during any period in which the United States has been at war, or since September 16, 1940. In cases where the original certificate was mutilated, the mutilated certificate must accompany the application. In cases where the applica­tion is for a certificate in a changed name, it must be accompanied by documentary evidence of the change, as well as the original certificate.
