12 TESTVÉRISÉG WE A RE NOW HOSPITALIZATION INSURANCE CERTIFICATES ATTENTION, MEMBERS AND CO-WORKERS! We are glad to announce that after completing negotiations with the Insurance Departments of the different states, we are in a position to offer a new plan of insurance to our members. In this connection we want to announce the following: 1. The Hungarian Reformed Federation of America from August 1, 1951, has been issuing Hospitalization Insurance to its members who are past 16 and under 55 years of age. New members — for the time being — are acceptable into this plan if, at the same time, they take out at least $500.00 worth of life insurance. In these latter cases the regular membership application also is to be filled out in addition to the application for Hospitalization Insurance. 2. The monthly dues for the Hospitalization Insurance are as follows: a) For men, $1.00 monthly, if they want $5.00 daily for hospitalization expenses, through 70 consecutive days; $1.75, if they want $10.00 daily for the same length of time. b) For women, $1.50 monthly for $5.00 hospital expenses through 70 consecutive days; $2.25, if they want $10.00 daily for the same length of time. Besides the daily hospitalization expenses, every member is entitled to those operation expenses which are enumerated in the rider or certificate, up to $150.00. In case of married women, a child birth aid is paid in the state of New York from the eleventh month following the date of the certificate, and from the thirteenth month in other states, in the second and third year of membership $25.00, and from the fourth year on, $50.00. In cases of Caesarean intersection, the double of these amounts is paid, that is, $50.00 and $100.00. 3. Besides the above outlined amounts of aid, every certificate carries a guarantee of $1,000.00 for accidental death for those who have $5.00 daily hospital expenses, and $2,000.00 for those having $10.00 daily hospital expenses. In cases of members accepted in the second class as “medium risks”, men pay 15 or 30 cents more monthly, and women, 10 or 20 cents more. Persons in dangerous or hazardous occupations are not acceptable for the Hospitalization Insurance. 4. For miscellaneous expenses (ambulance, X-Ray, etc.), $10.00 or $20.00 is paid, according to the amount of the daily hospital cost benefits. 5. It is desirable that three months’ dues be collected at the time the member receives the certificate. For six months’ advance payment, 1%, and for one year’s advance payment, 3% discount is allowed. The details of the provisions are as follows: a) The amount of the daily hospital expense is paid for every sickness or accident, if the member is confined in a lawfully operating hospital, from the first day, up to 70 (seventy) consecutive days for men and women alike. If aid is paid for 70 consecutive days, new aid can be given only after three months. b) In case of accident, the benefit is paid immediately after the date of the certificate. In case of sickness, the benefit is paid only THIRTY days after the date of the certificate. c) Members may be confined in any lawfully operating hospital in the United States and Canada. The $10.00 or $20.00 “miscellaneous expense”, however, is paid only if the member has spent at least five days in the hospital. d) The benefit checks are made out in the names of the members and not of the hospital, irrespective of membership in other similar Hospitalization Insurance plans. e) In cases of operation, the member is entitled up to $150.00 operation costs, in addition to the $5.00 or $10.00 daily hospital expenses. f) The cost of operation is paid not only once, but for every time the member is operated. In cases of accident, the member is entitled to aid immediately after the date of the certificates; in cases of sickness, after seven months following the date of the certificate, in gradually growing amounts, according to the conditions of the certificate. g) In cases of childbirth, aid is given in the state of New York from the eleventh month following the date of the certificate; in other states, from the 13th month, for not only one, but for every childbirth when the mother is confined in a lawfully operated hospital. h) In cases of childbirth, only the childbirth aid is paid and not the daily hospital costs. i) In cases of accidental death, every other benefit is paid in addition to the $1,000.00 or $2,000.00 benefit for accidental death. j) For securing Hospitalization Insurance, medical examination is not required. Monthly dues are paid to the branch secretaries who will send them to the Home Office with their regular monthly reports. k) To every certificate is attached a sick report, which is to be filled out and sent to Home Office through the branch in case of sickness or accident.
