Szemészet Supplementum I. 44 Refractive lens exchange in patients with presbyopia and myopia Nikica Gabric,1 Iva Dekaris,2 Ante Barisic,2 Damir Bosnar,2 Ratimir Lazic1 'Eye Clinic “Svjetlost”, Zagreb (Croatia) 2University Department of Ophthalmology, General Hospital “Sveti Duh ”, Zagreb Purpose: to report the clinical effects of bilateral implantation of the AcrySof Restor IOL in eyes of patients with presbyopia and myopia. Methods: AcrySof Restor IOLs were bilaterally implanted in eyes of 10 patients with presbyopia (N=20 eyes) and in eyes of 20 patients with myopia (N=40 eyes; -6.00 Dsph to -14.00 Dsph). The patients were between 42 and 68 years of age. 180 days after the operation, distance and near uncorrected visual acuity (UCVA), patient satisfaction, spectacle dependence, and visual disturbances were measured. Results: In the presbyopia group distance and near UCVA were excellent; one patient needed correction +1.00 dsph to achieve an acuity of 1.0, and all patients could read J1-J2. One patient required spectacle correction for distance and he wore the spectacles sometimes. Visual disturbances were not reported. In the myopia group about 80% of patients had distance and near UCVA which were the same as the preoperative best-corrected visual acuity (BCVA), while 10% had better and 10% worse distance and near UCVA than the preoperative BCVA. One patient reported blurring during night driving. Patients in both group were highly satisfied, and only one of them “was not sure” if he would the have the same IOL implant again. Conclusion: The AcrySof Restor IOL enabled good near and distant vision for presbyopes and myopes who underwent refractive lens exchange. The patients were highly satisfied. The AcrySof Restor IOL considerably reduced spectacle dependency and minimised visual disturbances. Filtrációs műtétet követően lágy kontaktlencseviselés mellett jelentkező ismétlődő chemosis és suffusio: esetismertetés ✓ Gonda Gyula, Makay Agnes Pest Megyei Flór Ferenc Kórház, Szemészeti Osztály, Kistarcsa Filtrációs műtétet követően Sofiens Multifocal típusú lágy kontaktlencse viselése mellett, visszatérő, mindkét oldali körülírt chemosissal és suffusioval jelentkező 49 éves betegünk esetét ismertetjük előadásunkban, aki lléve visel mindkét szemén lágy kontaklencsét -3.5D myopia miatt. Anamnéziséből kiemelendő a kétoldali kezdődő subcapsularis cataracta. Simplex típusú zöldhályog miatt 2005. februárjában bal ill. májusában jobb oldali trabeculectomia történt, mely után szemnyomása szemcsepp nélkül kompenzált maradt. A műtéti gyógyulást követően ismét elkezdett lágy kontaktlencse viselés során körülírt chemosist és suffusiot észleltünk több alkalommal, mely kezdetben differenciáldiagnosztikai problémát okozott. Eltérő átmérőjű, bázisgörbületű és anyagú lágy kontaktlencsére áttérve (Sofiens 66) a tünetek nem ismétlődtek. Az esetet azért tartottuk közlésre érdemesnek, mert legjobb tudásunk szerint az irodalomban ehhez hasonlót eddig nem közöltek. Esetünkkel a ritka, de érdekes differenciáldiagnosztikai problémát kívánjuk bemutatni.
