Szemészet First experiences with an aspheric IOL Entz B.B.,1-2 Auffarth G.,2 Becker K.,2 Holzer M.,2 Reuland A.2 'Semmelweis University, 2nd Department of Ophthalmology, Budapest 2Universitäts-Augenklinik, Heidelberg Background: Aspheric IOLs have been developed to optimise the optical characteristics of the intraocular lens (IOL). In patients the spherical aberration and the quality of retinal image changes with increasing age. We compared the functional results after implantation of one type of aspheric IOL, for patients in two different age-groups. Patients and methods: 43 patients (mean age 70.9±8.3 years) underwent uneventful cataract surgery with implantation of the Pharmacia Z9000 Tecnis aspheric IOL. The mean follow-up time was 6 months. Contrast sensitivity was tested with the Mesoptometer II instrument. For analysis, the patients were divided into two age-groups: group 1, younger patients with age <70 years (mean 62.4±6.8, range 43-69 years; n=16); and group 2, elderly patients with age >70 years (mean 75.9+2.3, range 70-83 years; n=27). Results: The preoperative visual acuity was similar in both groups (group 1, 0.4±0.3; group 2, 0.3±0.2). At 1 day postoperatively, the visual acuity of the younger patients was significantly better than the elderly ones (groupl, 0.9±0.2; group 2, 0.7±0.2), but at 6 months there was no marked difference (group 1: 0.8±0.1; group 2: 0.9±0.2). The contrast sensitivity was somewhat better for younger patients (2.5±0.6, compared to 3.3±1.2 for group 2). Conclusions: In the elderly patients, the aspheric intraocular lens appeared to provide some amelioration of the normally expected reduction in contrast sensitivity and quality of optical images due to increasing age. Further investigations involving a larger number of patients are necessary in order to properly evaluate this new lens and its potential advantages. Esetismertetés: a 38 éves korában extracapsularis hályogműtéten és PCL-beültetésen átesett beteg látási panaszai hét évvel a műtét után kezdődtek. Két év alatt több alkalommal spontán felszívódó elülső csarnoki vérzést észleltünk. További öt év panaszmentes időszakot követően a PCL spontán szubluxálódott, látásélességének periodikus csökkenését - fluoreszcein angiográfiával igazolt - cystoid maculaoedema magyarázta, mely nemszteroid gyulladáscsökkentő adására jól reagált. Következtetés: a hályogműtét szövődményeinek értékelésénél évtizedekben kell gondolkodnunk. Bajcsy-Zsilinszky Hospital, Ophthalmic Department, Budapest Background: We present a case of a monocular pseudophakic patient who after posterior chamber lens implantation has experienced periodic visual disturbances during the subsequent 14 years. Case report: Extracapsular cataract extraction and implantation of a posterior chamber lens was performed on a 38 year old male patient. He had good vision during the first 7 years follow-up. In the next two years however his vision deteriorated on several occasions; this was caused by haemorrhage into the anterior chamber, which reabsorbed spontaneously.
