Our experiences with LASER treatments

The authors summarise their experiences over six months with the use of the LASEK (Laser Assisted Intra-Epithelial Keratectomy) treatment procedure, and set these in a comparative context. Reviewing the relevant literature, an evaluation of the role of LASEK among the alternative excimer-laser refractive treatments will be given, together with its special indications, advantages and drawbacks in comparison with the LASIK and PRK procedures.

The authors present an overview of the outcomes of LASEK procedures performed on 30 myopic patients, and compare these with results for a similar group of patients treated using LASIK and PRK. Finally a video demonstration demonstrates the operating technique used by the authors.

Tapasztalatok HLA-tipizált corneák beültetésével

Célkitűzések: A HLA-tipizált corneák műtéti felhasználása és a betegek utánkövetése.

Módszerek: A Debreceni Szembankban 2002 októberétől vezettük be a keratoplasticára váró betegek és a donorok HLA-antigénjeinek vizsgálatát. Ebben az időszakban 20 HLA-A, -B, -DR tipizált cornea érkezett a Szembankba. Tekintettel arra, hogy a recipiensek és a donorok szűrése egy időben kezdődött, 8 betegnek tudtunk bizonyos fokú HLA- egyezést mutató corneát beültetni. Az egyezés mértéke: 4 lókusz egyezése 2 esetben (diagnózisok: rekeratoplastica, leucoma vascularisata corneae), 2 lókusz egyezése 2 esetben (keratoconus), 1 lókusz egyezése 4 esetben (2 pseudophakiás keratopathia bullosa, 2 rekeratoplastica) fordult elő.

Eredmények: A műtét után 6 hónappal valamennyi transzplantátum tiszta, átlátszó volt. Két beteg esetében endothelialis immunrejekció alakult ki, amely szisztémás szteroidkezelésre rendeződött.

Következtetések: A Debreceni Szemklinikán bevezettük és sikeresen alkalmazzuk a HLA-tipizálást, főként a fokozott rizikójú corneatranszplantációra váró betegek esetében. A HLA-rendszer extrém polimorfizmusa miatt annak valószínűsége, hogy azonos vagy közel azonos szövetet tudtunk volna beültetni a betegeknek, csekély volt. Ehhez egy nagyobb számú beteget tartalmazó lista, azaz országos várólista kialakítása szükséges.

Experiences with HLA-matched corneal transplants

Background: In our Department HLA screening for corneal transplants was introduced in October 2002. Since then, 20 corneas have been HLA-typed (HLA-A, B, and DR) for the Debrecen Eye Bank. Screening for donors and recipients started at the same time, and we have performed 8 HLA-matched corneal transplants. We report our experience with the surgical use of HLA-matched corneas, and the follow-up results with patients.

Results: The degree of matching in our 8 transplants was as follows: 4 identical loci in 2 cases (rekeratoplasty, corneal vascularisation), 2 identical loci in 2 cases (2 keratoconus), and 1 locus in 4 cases (2 pseudophakic bullous keratopathy, 2 regrafts). After a six month follow-up period all grafts were clear and transparent. In two patients endothelial immunological rejection was observed, which was successfully treated with systemic steroids.
