BASIC HOSPITAL, HOME HEALTH, AND NURSING HOME BENEFITS: (1) Eligibility: All persons age 65 and over.* Hospital and home health benefits will begin July 1, 1966. Nursing home benefits will begin Jan. 1, 1967. (2) Benefits: • 60 days inpatient hospital care and services per spell of illness. Patient pays first $40 of charges. 30 days additional care, with patient pay­ing $10 per day. Psychiatric hospital services are included, but with a lifetime limit of 190 days care. • Outpatient hospital diagnostic services fur­nished by the same hospital during a 20-day pe­riod. Patient pays a $20 deductible for diagnostic services, and 20% of costs after the deductible is met. • Post hospital home health services for up to 100 visits after discharge from a hospital (after 3 or more days stay) or an extended care facility (nursing home), and before the beginning of each new spell of illness. • Up to 20 days in each spell of illness of post hospital extended care in a facility (nursing home) having an arrangement with a hospital, if the pa­tient is transferred after at least a three-day hos­pital stay. Thereafter, patient may receive up to 80 additional days of care in the extended care facility, with the patient paying $5 per day. (3) Definitions: A “spell of illness” begins when the patient enters a hospital or extended care facility and ends when he has not been an inpatient in a hospital or extended care facility for 60 conse­cutive days. * After January 1, 1968, these benefits will be available ONLY to persons covered under the Social Security Act, and under the Railroad Retirement Act. ■^^ÖLÜNIASY MEDICAL AND HEALTH SERVICES PROGRAM: (1) Eligibility: All persons age 65 and over may voluntarily enroll for these benefits. Benefits will begin July 1, 1966. Persons age 65 before January 1, 1966 may en­roll until March 31, 1966. Persons age 65 after December 31, 1965 will have an enrollment periods of 7 months beginning 3 months before they attain age 65. Future gen­eral enrollment periods will be from October to December 31 in each odd year, beginning in 1967. (2) Benefits: from low-income families and neighborhoods. • Special grant will be made for two fiscal years to implement and follow-up plans to combat mental retardation. • Federal shares of payments under public as­sistance cash payment programs will be increased by about $2.50 per month per needy aged, blind and disabled, and about $1.25 per month for needy children. E. INCREASED OLD AGE, SURVIVORS AND DISABILITY BENEFITS: (1) 7% across-the-board increase in monthly benefits, with a minimum increase of $4. This cash benefit increase is retroactive to January 1, 1965. • Physicians’ and surgeons’ services furnished in a hospital, clinic, office, in the home or else­where. • Out-of-hospital psychiatric care maximum per calendar year of $250 or 50% of expenses, which­ever is smaller. • Home health services up to 100 visits during each calendar year. • Additional medical and health services in­cluding: —diagnostic X-ray. —laboratory and other tests. — X-ray, radium and radioactive isotope therapy. —ambulance services. —surgical dressings. —splints, casts, prosthetic devices, rental of durable medical equipment, etc. (3) Patient Payments: • Patients pay the first $50 of annual costs of the above benefits; thereafter, patient pays 20% of costs above $50, and the program will pay 80% of the costs. C. EXTENSION OF MEDICAL CARE TO NEEDY INDIVIDUALS AND FAMILIES: (1) Eligibility: Applicants must prove their in­come and property is insufficient to meet their needs . . . i.e., they must be recipients of public welfare or prove they are “medically indigent.” (2) Benefits: • By January 1, 1970 States must provide in­patient and outpatient services, lab and X-ray services, skilled nursing home services and phy­sicians’ services in the home, the office, in hos­pitals or skilled nursing homes. This range of services is the prerequisite to Federal participa­tion in medical care programs offered by the States. Provision of other medical services is at the discretion of each state. No deductibles, cost sharing or such charges may be made by State agencies for hospitalization. • State programs will be required to provide assistance to the needy elderly in meeting the de­ductible charges of the basic hospital, home health, and nursing home benefit program. • States may assist the elderly in meeting de­ductibles and cost sharing features of the volun­tary medical and health services benefit program (with such cost sharing to be related to the re­cipient’s income or resources). • Exclusions from Federal matching under as­sistance programs for aged patients in TB or men­tal disease institutions are removed. (3) State participation: Unlike the basic and voluntary Medicare pro­grams, this is a state-administered program with funds becoming available on a Federal State matching basis. D. SPECIAL PROVISIONS TO EXTEND AND EXPAND EXISTING HEALTH PROGRAMS: • Federal amounts authorized for maternal, child health, and crippled children’s services will be, raised, in order to enable States to make these services available to children in all parts of the States by July 1, 1975. • Federal funds will be made available to train personnel for health and related care of crippled and mentally-retarded children. • Special 5-year project grants will be made to State health agencies, State crippled children’s agencies, medical (and dental) schools, and teach­ing hospitals to provide comprehensive health care and services for preschool and school-age children Illustrations of current maximum cash benefits:* Old Age Benefit Old Law New Law (or Disability Benefit) $127.00 $134.90 Wife’s Benefit (based on husband’s earnings**) 63.50 67.90 Survivor’s Benefit (Aged widow, widower or parent) 104.80 123.70 Widow and Two Children 254.10 312.00 *Two factors have the effect of reducing these maximum benefits: (1) All workers (except some persons retiring after age 65) will receive about $4 below the maxi­mum shown, because in figuring their average earnings, they must include some years when the Social Security tax base was on less than $4,800 annual earnings. (2) Some workers will not have up to $4,800 earnings in some years which must be used in computing average earnings. **Some wives, who have worked will be entitled to a higher benefit based on their own earnings than they would be entitled to based on their hus­band’s earnings. (In such cases, the higher benefit is payable.) NOTE: Maximum benefits in the new law will be higher than the current new maximums shown as future retirees compute Social Security. For example, Old Age & Disability benefits will even­tually reach a maximum of $168.00 (as compared to $135.90 currently).
